If you are ordering  more than 10 t-shirts by mail, please email me to check on availability. Please enter the number of shirts you want by selecting the quantity from the drop down menu next to each t-shirt size, and then print out this form and mail with your check or money order to: Frank Mundus, P.O. Box 941, Naalehu, Hawaii 96772. For orders over $50.00, I provide free insurance via US Postal Service Insured Mail. Sales tax is charged only on orders sent within the state of Hawaii.**Hawaii residents, please add 4.000% state sales tax. Will ship only within the U.S.

T-shirt and Sweatshirt shipping rates:
1 t-shirt or sweatshirt $4.55;
2 t-shirts or sweatshirts $6.95;
3-4 t-shirts or sweatshirts $9.95;
6 t-shirts or sweatshirts $12.50.

Items paid for by money order will be shipped immediately. If you pay by check, please allow 7-10 days for it to clear with the bank.
